Q. I have patients previously diagnosed as Asthma who are now classified as COPD. Should I keep them on inhaled steroids? A. If you have a strong suspicion that your patient has COPD rather than asthma (smoking history, progressive shortness of breath), then you should slowly reduce the dose of inhaled steroids (over several months - e.g. 25% reduction every 2-3 months). In most patients with COPD you will be able to withdraw the inhaled steroid completely, without any increase in symptoms or change in lung function. Q. What is a recommended 'steroid trial' for COPD? A. The most frequently used regime for a steroid trial is to give oral prednisolone (40mg daily in a single morning dose) for 2 weeks. It is best to make measurements of peak expiratory flow (PEF) twice a day for 2 weeks before starting oral steroids in order to get a baseline value and then during the treatment period. FEV, should be measured at the beginning and end of the baseline period and after the inhaled steroids. A steroid trial is considered positive if FEV, or mean PEF increase by more than15%, when a diagnosis of asthma is made. It is important to remember that some patients will have asthma and COPD at the same time. Q. What is the role, if any, for oral steroids in COPD? A. Oral steroids are used in a formal trial of steroids and may be useful in acute exacerbations of COPD. They should not be used as a maintenance treatment because of the side effects of long-term steroid therapy. Q. With a newly diagnosed COPD patient should I initiate steroid therapy, if so how? A. If a patient has COPD with no evidence of response to oral steroids (negative steroid trial) then inhaled steroids should not be used. It is possible that inhaled steroids may alter the accelerated annual rate of decline in lung function with age. Q. How will I know if a COPD patient is benefiting from steroid therapy? A. The best way to determine whether inhaled steroids are beneficial is to slowly reduce the dose. If FEV, or mean PEF show no deterioration, the inhaled steroid can be completely withdrawn. Q. Will COPD patients need to be on long-term steroid therapy? If so, what about side effects? A. Only patients who have shown a positive response to oral steroids (steroid trial) should be on inhaled steroids.

Computer and Grades
This chapter presents the background of the study, problem and its significance, and the scope and limitation of the study. Background of the Study In this Information Age we are able to process or transmit, sort and retrieve data or information between one computer to another. With this, it makes the communication and different task done easy and less time consuming and hassle free. Like in schools, offices this brings a big help. In schools especially in colleges/ universities it is consist of different people and machine that work together to achieve one common goal.One of this goal is the transferring of data or information of computed grades of the student on how they performed academically in their different respective subject. On latter days, instructors manually computing the grades of their students then input it in computer, print it for them to have the hard copy. One is for their own then the other copy for the registrar or dean whom they passed the reports of the grades. But with this grading system, they can compute or tabulates the grades of their students computerized, easily and efficiently, less paper works and they can pass it through computer to the registrar and dean.And most important is that the student can quickly response if he have a problem with his grades because of the system SMS notification. The student will receive a SMS notification if he is needed to comply or talk with his respective instructor about his academic performance or grades. This will help the student that always have the reason of it was too late when they know that they failed on that subject so they were not able to comply or talk with their instructor. Objectives of the Study The main objective of the study is to minimize the task of the instructor.Registrar. This study will benefit and help the registrar to monitor the student list and information with the grades coming from their instructors. Dean. This study will benefit and help the dean of this college to m onitor the class standings and per term submission of grades of the students by the instructors. Future Researchers. Usable ââ¬â this is very useful every time that the instructor need to compute the grade of each student because this system can stored data on the database which is secured and the losses of each data will be lessen compared to the manual computation of grades. 4) Easy to Use ââ¬â it is understandable to use by the user because of the separation of interfaces of who are logged on in the system, the proponents separates the interface according on the task of the user. 5) Informed ââ¬â every end of the term the students will receive a notification from this system according t o their status to be able for them to know about the status of their grades.The Grading System with SMS Notification has three interfaces according to the profession of the user. The instructor, has an interface to create, update and view the grades of each student. The Registrar, has an interface to view the grades of each student every end of the semester encoded by the instructor. The Dean, has the interface to view the grades of the student every end of the term. The Grading System with SMS Notification can send information according to the registered phone umber of the student on the system whether it is a personal phone number or the phone number of their guardian.

The Baylor University Concert Jazz Ensemble Directed By...
I attended the Baylor University Concert Jazz Ensemble directed by Alex Parker on Tuesday, November 10th. This ensemble consisted of saxes, trombones, trumpets, drums, a piano, and bass. The concert showcased many individual talents by including soloââ¬â¢s or duetââ¬â¢s in every song. The program consisted of seven songs with no intermission. The pieces I selected were ââ¬Å"Big Dipperâ⬠by Thad Jones and ââ¬Å"Undecidedâ⬠by Frank Mantooth. The composition ââ¬Å"Big Dipperâ⬠by Thad Jones consisted of a duet by Hunter Horby playing the tenor sax and Ryan McArthur playing the trumpet. The tone of the piece was in major. It had a happy feel to it and made you want to get up and dance. It was moderato in tempo, meaning it was an average pace of beats. It kicked off with a quick crescendo from mezzo piano, or medium soft, to forte, or loud. The tenor sax and the trumpet had the melody and the rest of the instruments were the background music for most of the piece however, the rest of the ensemble had their time to shine with the melody a couple of times. I felt as if I was in the 1950ââ¬â¢s listening to swing music and jazz. The texture felt dancelike and brassy. Jones incorporated many accents and staccato markings throughout the composition. The song had many contrasts with pitches and dynamics. Most of the crescendoââ¬â¢s and decrescendoââ¬â¢s seemed to be directly related to the amount of instruments being played. Symbolism of the poems Essay Several motifs used by Keats and Shelley enhance the symbolism of the poems. The poets use several motifs to their command and therefore succeed in revealing their innermost thoughts to the readers. For instance, in Ode On A Grecian Urn, the urn is symbolic to the time which forever stays stagnant. Keats wants to compare the stillness in the world of art to his life. Since he is living on borrowed time, Keats is aware that every moment of his life counts. However since he knows that has a fast-fading life he pictures the vase not only as a piece of art but also as his idol. In a way Keats wants to imitate the stillness in the vase because the pictures of the two lovers cannot fade away. The nightingale is a symbol of freedom and ancestral royalty at the same time. Keats has presented the nightingale to his readers in a very favourable light. This is because according to Keats, the bird had served royalty and has forever existed. He sees the nightingale as an image of infinity. In a way, Keats pines to be in place of the bird because it expresses itself in an extremely graceful and melodious way. To Keats, the season of autumn is also figurative as it is like an ideal dream. Keats has depicted autumn as a season which is forever conspiring about how to load and bless life. We notice that it is not really nature that needs the blessings of autumn but Keats own life. The fruits which have been filled with a ripeness to the core, pose a paradox to Keats life which is supposed to be unfruitful and unproductive. Even the west wind used in Shelleys poem symbolizes the winds of change. Since Shelley has the enthusiasm to change the world, he summons the autumn wind to help him being over this change. Shelley wants to recognize himself as the master of the seasons. He is like a lion tamer who wants the ferocious winds to respond to his command. To Shelley the skylark is symbolic to freedom and symphony. In the poem, Shelley marvels the grace and elegance of the skylark. According to Shelley, the skylark showers a rain of melody. The skylark can be compared to the bird in Shelleys own mind which wants to venture to unseen areas and which wants to explore the different highs and lows of human nature. The skylark can also be compared to a symbol of life itself, which, if taken positively and with the right attitude is like a star of heaven. Thus to Shelley, the skylark is not only a motif of life but also a symbol of unexplored lands and unventured areas. The vivid use of imagery throughout the different poems attracts the readers interest and denotes the writers creativity. All of the poems are packed with imagery which not only shows their uniqueness but also the intensity and sophistication in the writers. Keats has used the imagery of intoxication to add a dreamy effect to his poems. Keats refers to the fume of poppies in To Autumn and to the hemlock and the draught of vintage in Ode To A Nightingale. This may suggest the clouded mind of a person drunk with his own thoughts. Also, it may signify the clouded mind of a patient suffering from tuberculosis. Keats also may have used these symbols as they signify his ultimate intoxication that awaits him-namely death. Along with these, there is an imagery of disease and sickness all throughout in Keats poems. Both, Shelley and Keats have used alliteration to their advantage. Shelley calls upon the wild west wind to spread his ideas of change. These words demonstrate Shelleys plea to the west wind to lift him as a wave, a leaf, a cloud. Alliteration is also used by Keats in his poem Ode To A Nightingale, to highlight his grief. The self-same song of the nightingale shows that it has existed for generations and the same harmonious song has even found a way to the sad heart of Ruth. Keats uses Ruth as the typical icon of moaning. She is not only a symbol in his poems but also a metaphor. Keats compares himself to the moaning heart of Ruth and suggests that since the nightingale has cured the sadness of Ruth with its songs, it may also be able to relieve Keats of his misery. Keats has also used an image of Bacchus and his pards as a metaphor of intoxication. Since Bacchus is the Roman God of wine Keats in a way is requesting him to assuage his own anguished soul by giving him the intoxication to forget his woes. Both Keats and Shelley use personification to develop the presence of nature and give it some attributes. Keats personifies the season of autumn which is the close-bosom friend of the maturing Sun. he has sketched a picture of autumn in the minds of the reader and by looking at the season the picture of the mind revives again. Both the poets use irregular end rhymes in the poems to give it a continuous tone and flow. In addition to the rhymes, Keats also uses a variety of onomatopaeic sounds, in particular the O sounds. In Ode To A Nightingale he combines the rhyming words known and groan along with the word sorrow. These words make it seem as though life for Keats is a recurrent, monotonous drone. Even Shelly uses onamatopaeia to summon the wind.
